A septate uterus is a common type of congenital uterine anomalyand it may lead to an increased rate of pregnancy loss. The main imaging differential diagnoses are arcuate uterus and bicornuate uterus. It is classified as a class V Mullerian duct anomaly.
A rare case of Mullerian anomaly in a year-old woman who presented with recurrent pregnancy loss and was found to have a longitudinal vaginal septum with cervical duplication and two endometrial cavities separated by a complete septum. The diagnosis of this unusual anomaly was aided by magnetic resonance imaging and was confirmed by hysterolaparoscopy. It was concluded that hysteroscopy aided in the diagnosis and treatment of this Mullerian defect with the highest quality of precision.
This paper describes a rare case report of a patient with complete septate uterus with double cervix and longitudinal vaginal septum, submitted to the vaginal septoplasty for dyspareunia, progressing to clinical improvement. Generally, these changes are associated with genetic mutations, developmental disabilities, or environmental causes that have an effect on embryonic development stages [ 1 ]. However, septate uterus with cervical duplication and longitudinal vaginal septum is not described by this classification [ 2 ].

The wall of tissue can run vertically or horizontally, dividing the vagina into two sections. However, some women with a vaginal septum never have any symptoms. A longitudinal vaginal septum LVS is sometimes called a double vagina because it creates two vaginal cavities separated by a vertical wall of tissue.
A vaginal septum is a vaginal anomaly that is partition within the vagina ; such a septum could be either longitudinal or transverse. In some affected women, the septum is partial or does not extend the length or width of the vagina. As a result, there may appear to be two openings to the vagina. A complete transverse septum can occur across the vagina at different levels.
